The Community Support Programs are comprised of Mobile Treatment Teams, Community Support Teams, and Residential programs. The manager of each program participates in an agency wide disposition process that determines program eligibility and ensures the referral and placement of each eligible individual on the appropriate team.

Clients assigned to a program receive a case manager and psychiatric services. Based on assessment and individual programs, a client may receive services from other disciplines such as nursing, counseling, and rehabilitation services. All staff work with clients to encourage the full participation in the assessment and treatment planning process to promote recovery.

The mission of the Community Support Program is to provide a comprehensive array of clinical and rehabilitative services in support of an individual’s efforts to recover and rehabilitate from mental illness. The goal is to enable clients to stabilize their functioning, re-establish healthy patterns of living and reintegrate into the everyday lives of their families and of their communities.

Services available to the clients of a Community Support Program include: assertive outreach; clinical case management; medication management; psychiatry; psycho-education; nursing; community integration; resource acquisition; advocacy and access to the community’s natural support system.

The Community Support Program serves adults with a serious and persistent mental illness who have been determined eligible via our comprehensive assessment and case disposition process.

Participation in a Community Support Program is determined by eligibility criteria promulgated by the Rhode Island Department of Mental Health, Retardation and Hospitals. Eligibility criteria is based on diagnosis, treatment history and functional impairments.
