| CASE MANAGER - CHARLESTOWN HOUSE - 1st and 2nd Shift |
Job Description
Provide case management services to individuals with severe and persistent mental illness living in a residential setting. Duties will include providing support and assistance with daily living activities, therapeutic groups and assisting clients in the community.
Position is full-time - 40 hours per week
Sunday through Thursday - combination of day and evening shifts

Job Description
Residential Nurse works together with Residential Staff and Psychiatrist to provide rehabilitative recovery-based services to group home adult clients.
Responsibilities include, but are not limited to: medical assessment, vital sign monitoring, education and dispersion of psychiatric and medical medications, MOT pack preparations, and injections. Also participates in treatment planning process for all residential clients. Nurse will communicate with hospital nursing staff when clients are hospitalized and in the discharge process to ensure medication continuity upon discharge.
This position may travel between Westerly and North Kingstown regions.
Full-time Position - 37.5 hours Monday - Friday (Flexible, if needed)
